Description
A Vacancy exists for a Social Worker at the Rehabilitation Unit at Life Entabeni Hospital. The Life Rehabilitation unit is part of an expanding network of rehabilitation units within Life Healthcare. The post holder will report to the Therapy Services Coordinator and will be expected to provide holistic social work intervention.
Critical Outputs
Provide social work clinical services to all patients admitted to Life Rehabilitation Unit, including on some weekends and public holidays.
Conduct assessments of the patient, family and home environment, provide counseling services and coordinate discharge planning to patients with e.g. neurological disorders such as TBI, CVA, SCI, orthopaedic conditions or multi-morbidity conditions such as Guillain Barre, etc. in collaboration with other therapy disciplines.
Educate patients and their families about the patient’s condition and explore discharge and/or placement options.
Provide supportive services to patient families.
Identify and coordinate carer training.
Engage with employers when indicated to explore return to work opportunities and reasonable accommodation.
Contribute to weekly team and family meetings.
Keep accurate and timeous records and reports.
Effectively respond to queries and handle accordingly.
Professional therapy service implementation in accordance with Life Healthcare protocols and procedures.
Function effectively as a part of an interdisciplinary team to meet the requirements of quality patient care and patient experience
Attend and contribute towards Continuous Professional Development activities of the unit.
Supervise and train of university students, as required.
Promote and maintain sound and productive relationships with referral sources and stakeholders in the field of Rehabilitation.
Requirements
Degree in Social Work and relevant registration.
Experience in hospital-based social work.
Experience in multi-disciplinary care.
Patient advocacy.
Local resource knowledge an advantage.
Commitment to quality patient care and customer satisfaction.
Current COVID-19 vaccination.
